Looks like someone's iPhone is telling them mail failed to send when it did send. I got that about half the time I tried sending mail. If I had to guess, it's that if mail isn't sent within a certain period of time, it's called a failure. This opposed to every other mail program in the history of mail that calls it failed if it never sent.
